• Full Name as per NRIC (registered with Allianz We Care Community)
• Mobile number (registered with Allianz We Care Community)
• Patient’s condition
• Address (From & To, within KL & Selangor)
• Date & Time
- Confirmation of the discounted non-emergency ground ambulance transfer service will be sent to the first 50 Allianz We Care Community members upon verification based on a first-come-first-served basis.
- This benefit is only valid within KL & Selangor area only. Prior booking is required and it is subject to availability.
- Allianz We Care Community members must email the required details to to arrange for an appointment.
- This benefit is provided to the first 50 Allianz We Care Community members based on a first-come-first-served basis.
- Each Allianz We Care Community member is limit to one (1) usage of the discounted non-emergency ground ambulance transfer service only.
- Each enquiry will be accessed by Regal Ambulance's experienced Emergency Medical Dispatcher. Regal Ambulance reserves the right to refuse any clinically unnecessary ground transfer request.
- By sending the email request, Allianz We Care Community member acknowledges that he/she has read and agreed to the Regal Ambulance's .
