The team is ready to provide you with skilled and specialised assistance, focusing on stroke, diabetes,  heart and cancer-related post-hospitalisation via phone call or WhatsApp message.

With the ‘Call For Care’ helpline, you can get :

  • Medical guidance – for information and advice from healthcare professionals
  • Resource referrals – access to relevant support services, community resources and healthcare facilities that can aid in the post-hospitalisation journey
  • Coordination of services –with various healthcare professionals for a seamless transition from hospital care to post-hospitalisation support.
