  • Allianz General PUI / PUS Claim Form
    Download form
  • Copy of Home Surveillance & Observation Order from MOH and/or official documents from MOH / hospital as proof on fulfilling PUI / PUS criteria
  • Copy of NRIC (Both sides)

  • Allianz General Cash Relief Claim Form

  • E-payment Authorisation Form
    Download form
  • Copy of admission bills / invoices, medical report / discharge summary from the government / public hospital or quarantine and treatment centre managed and operated by the government, lab tests performed for COVID-19
  • Copy of NRIC (Both sides)

  • Copy of COVID-19 test result
