a) E-policy is only applicable for policies issued after 6 March 2017 and customers who opted for e-policy during policy application.

b) No processing fee is required for this policy

Replacement/Duplicate Medical Card

a) Duly signed letter from Policyholder

Change Payment Mode or Frequency of payment

a) Request For Change Form

Application Of Policy Loan

a) Policy Loan Application form
b) Policy Loan Agreement form (2 original copies)
c) Proof of Age Policy Owner / Assignee Certified by Commission of Oath or Allianz Customer Services Officer.
d) Proof of Age Life Assured Certified by Commission of Oath or Allianz Customer Services Officer.

Withdrawal of Cash Dividend, Regular Cash Payment or Survival Benefit

a) Discharge Receipt

Reduced Paid-Up

a) Application for Reduced Paid-Up Form
b) Proof of Age Policy Owner / Assignee Certified by Commission of Oath or Allianz Customer Services Officer.
c) Proof of Age Life Assured Certified by Commission of Oath or Allianz Customer Services Officer.
d) Conversion will only take place on Policy Anniversary.

Extended Term Insurance

a) Application for Extended Term Insurance Form
b) Proof of Age Policy Owner / Assignee Certified by Commission of Oath or Allianz Customer Services Officer.
c) Proof of Age Life Assured Certified by Commission of Oath or Allianz Customer Services Officer.
d) Conversion will only take place on Policy Anniversary.

Surrender of Policy

a) Application of Surrender form duly completed
b) A copy of Policy Owner/ Assignee NRIC
c) A copy of Life Assured NRIC
d) Original Policy Contract

a) Signature of Policy Owner/Assignee

c) Signature of Witness
d) E-Payment Section duly completed
e) Customer Declaration Section duly completed

Our Surrender Process:
a) Submit complete document to Allianz Life Branch
b) Allianz receives document and process surrender
c) Surrender approved (Approved within 4 working days for standard cases)
d) Surrender payout via E-Payment (Payout within 5 working days)
