Claims Made Easy

At Allianz, we are committed in ensuring that your insurance claims is made easy every step of the way so you can continue focusing on what's truly important in your life.

What to do during an accident

  • Call Bomba (for fire loss)
  • Make a police report (for theft loss)
  • Take photos of damaged items
  • Notify Allianz or your agent
  • Submit the required documents to Allianz / Agent / Adjusters

Note: Do not remove or dispose-off damaged items before Allianz’s investigation

* This is applicable to property claims (except for claims from Fidelity Guarantee, Marine and Liability policies).

Building/House Danage

  • List detailing the description, quantity and unit cost claimed
  • Historical purchase invoices in substantiation of the items claimed
  • Quotations for repairs / replacements in substantiation of the items claimed
  • Police Report (for theft loss)
  • Fire Brigade Report (for fire loss)
  • Technical report to confirm the cause of damage (if applicable)
  • Commercial invoice, original marine policy or certificate of insurance, original bill of lading or airway bill, original delivery note or consignment note, survey report and photos to show the extent of damage and other supporting documents (for Marine claims)
  • Duly completed E-payment form

*Applicable to Householder/Houseowner, Smart Home Cover, Fire, Marine, Fidelity Guarantee, All Risks, Burglary, Machinery Equipment, Engineering

Travel Claims

  • A brief circumstances of the loss
  • Relevant documentation in support of the items claimed e.g. reports, bills, receipt, medical report, photographs, travel itinerary etc
  • Duly completed E-payment form

Liability Claims

  • A brief circumstances of the loss
  • All supporting documents e.g. incident reports, letter of demand, writ of summon, repair/replacement receipts, medical bills etc

Personal Accident/ Group Personal Accident / Workmen Compensation / Foreign Worker Compensation Scheme claims

  • A brief circumstances of the loss
  • Death certificate
  • Post Mortem report (if any, for death claim)
  • Medical report
  • Medical Leave Certificates
  • Original itemised medical bill and receipts
  • Police Report (for motor vehicle accident)
  • Driving license (if you are driving / riding)
  • One month salary slip (for GPA, WC and FWCS)
  • Duly completed E-payment form

Hospital & Surgical claims (for reimbursment and non-cashless policies)

  • Completed claim form
  • Medical Report
  • Original itemized bills and receipt
  • Original medical invoice

  • Copy of passport depicting the entry and exit dates (for overseas treatment)
  • Duly completed E-payment form
